The closing date is 19 May 2026
Cornwall Partnership NHS Foundation Trust is leading a bold transformation in community and palliative end‑of‑life care across the county. Our vision is simple yet powerful: to ensure people nearing the end of life experience a peaceful, dignified death in the place of their choosing.

* Expanding services in collaboration with the Macmillan Social Investment Fund and local partners:
* 24/7 Rapid Community Response and Care Service
* Dedicated End of Life Care phone line for public and professionals
* Strengthened voluntary sector partnerships and Hospital@Home virtual service
We have opportunities available across various localities within the county, offering flexibility in base location depending on service need and candidate preference. A requirement is to cover Cornwall and the Isles of Scilly.
