 
        
        1. Demonstrate good clinical knowledge and practice at all times and be accountable for own actions.
 2. Triage referrals to ensure correct referral pathways have been followed and that patients are clinically suitable. (This is in line with Referral and Treatment Criteria, Directory of Services and local policy)
 3. Record clinical reason for acceptance or rejection against guidance for each referral.
 4. Attend monthly NHS review meetings to ensure contractual arrangements are being met and keep up to date with any changes
 5. Liaise with multidisciplinary teams and identify complex/ exceptional cases that require escalation or further clinical review.
 6. Work with the Clinical Nurse Manager to ensure safe patient selection
Person Specification:
 7. Be a registered Nurse (NMC registration essential)
 8. Have post-registration clinical experience ideally, across medical and surgical specialties
 9. Experience in clinical assessment, triage or referral management is preferred but not essential.
 10. Excellent analytical, communication and decision-making skills
 11. Ability to work autonomously
 12. Maintain high standards of clinical documentation and data accuracy
 13. Have knowledge of clinical governance and its application
 14. Demonstrable knowledge of current nursing and clinical developments
