Overview
Join to apply for the Team Leader – LD Provider Services Learning role at City of York Council. Location: 77 Flaxman Avenue, a short stay and overnights establishment for adults with a Learning/Physical Disability and/or Autism, delivering person-centred/strengths-based care while promoting independence and choice in daily activities. This role has a strong preventative focus to support individuals with Learning Disabilities and/or Autism to live fulfilled lives while attending day services, short stay and overnights.
The post holder will work in an integrated way across a range of service settings within York, pursuing valued opportunities for Adults with Autism and/or a Learning Disability with complex health needs. You will take the lead for writing Care Plans, Positive Behaviour Support Plans, Risk Assessments and all customer plans, and you will have responsibility for the safety and security of the council buildings you work in.
Responsibilities
* Lead, support, guide and monitor Support Workers on shift.
* Develop and maintain Care Plans, Positive Behaviour Support Plans, Risk Assessments and other relevant plans for customers.
* Work in an integrated way across service settings to promote independence, choice and person-centred care.
* Ensure safety and security of council buildings and contribute to safeguarding and welfare.
Qualifications and requirements
* Strong Team Leader skills and the ability to supervise and develop staff.
* Competency to converse and provide advice and guidance to members of the public in spoken English at CEFR level C2 (as part of interview assessment).
* Eligibility to undertake an enhanced Criminal Record check via the Disclosure and Barring Service.

Benefits and working patterns
* Sleep in rate £42.12.
* Shift work enhancements = 0.68 per hour on top of salary.
* Weekend work = 10% of hourly rate.
* Evening work enhancement from 7pm – finish time = 10% of hourly rate.
