To provide professional leadership and support to the Enhanced care/prevention of Delirium (POD) team based within the acute Trust. By working collaboratively with clinical areas utilising the available resources to provide an enhanced level of patient centred care. To support the dementia and delirium training, bringing a skilled workforce in dementia and delirium to the organisation., The post holder provides senior clinical, professional and operational leadership for dementia and delirium services, focusing on enhanced care, delirium prevention and engagement initiatives across the Trust. The role leads the Enhanced care team (ECT) to deliver high-quality, person-centred and dignified care for people living with dementia or at risk of delirium.
The post holder leads the A-level and further education student enrichment programme, working with local colleges to recruit, induct and train students, and supports the development of the volunteer workforce across clinical areas. Advanced clinical expertise are required to assess patients with, or at risk of, delirium and requiring enhanced therapeutic observation, while supporting ward teams to develop individualised care plans and ensuring effective deployment of the ECT.
Working in line with national and local guidance, the post holder acts as a clinical role model, leading Trust-wide education in dementia, delirium prevention, the Butterfly Scheme and John's Campaign. The role involves close collaboration with multidisciplinary teams, community services and third-sector partners to improve continuity of care.
The post holder also holds managerial responsibility for recruitment, performance reviews, rota planning, workforce management, risk and complaint handling, service evaluation and development, ensuring care is evidence-based and delivered in line with the NMC Code.
