What you will do
* Conduct comprehensive, person-centred assessments in clients\' homes or community settings, ensuring a holistic approach to care.
* Develop and implement tailored rehabilitation plans to support functional independence and recovery, making a direct difference in clients\' lives.
* Recommend and arrange assistive equipment and minor home adaptations to promote safety and autonomy, ensuring clients thrive in their own homes.
* Support timely hospital discharges and reduce avoidable admissions through proactive early intervention strategies.
* Collaborate with GPs, nurses, physiotherapists, social workers, and voluntary agencies to deliver integrated, multi-disciplinary care.
* Independently manage a varied caseload, using advanced clinical reasoning and prioritisation skills to maximise outcomes.
* Supervise junior OTs, assistants, and students, playing an active role in their growth and development within the field.
* Participate in service audits, innovation projects, and continuous improvement initiatives, helping to shape the future of community therapy services.
* Maintain accurate clinical records and reports in line with legal and professional standards, ensuring accountability and transparency.
* Educate clients, carers, and community partners on strategies to maintain health and independence, empowering communities through education.
