Band 4 Associate Practitioner - Gravesham Community Hospital, Gravesend
We have an exciting opportunity for a Band 4 Associate Practitioner to join our dedicated team on Sapphire Ward at Gravesham Community Hospital. Working under the supervision of a Registered Nurse, you will deliver safe, high-quality care to patients in a rehabilitation setting, following national and local priorities, Trust policies, and Nursing & Midwifery Council standards. This role requires NMC registration and is ideal for someone with health or social care experience who is ready to develop their clinical skills, work as part of a supportive multi-disciplinary team, and make a genuine difference to patient outcomes.
Responsibilities
* Deliver patient care in accordance with Trust policies, procedures, and professional standards as delegated by the Nurse in charge
* Undertake technical nursing tasks such as dressings, glucose monitoring, and other delegated clinical procedures
* Assist with patient assessments and implement care plans set by the Registered Nurse
* Accurately document all care provided using EMIS and ensure confidentiality is maintained at all times
* Recognise and report changes in patient condition promptly to the Nurse in charge
* Support administrative and clerical tasks on the ward, including ordering stock and equipment checks
* Participate in audits, surveys, and multi-disciplinary meetings
* Support the induction of new staff and students
* Engage in personal development through training, supervision, and appraisal
* Promote the safety and wellbeing of patients, staff, and visitors, ensuring a safe working environment
* Adhere to information governance, safeguarding, infection control, and health & safety requirements at all times
The Ideal Candidate
* Is NMC registered and committed to maintaining professional standards
* Has previous experience in a health or social care setting (rehabilitation experience desirable)
* Can work well within a team but also organise their own workload without direct supervision
* Has strong communication skills and a compassionate, patient-focused approach
* Is flexible to work across a seven-day rota
* Is competent in IT systems and willing to learn EMIS
* Can undertake a range of clinical skills, with desirable experience in venepuncture, catheter care, stoma care, and wound management
* Can assess patient needs and implement care under the direction of a Registered Nurse
* Can cope with occasional exposure to distressing or emotional situations while maintaining professionalism
* Is committed to personal and professional development
