As a Case Coordinator, you will support individuals experiencing or at risk of homelessness to maintain their accommodation and develop independence. Your responsibilities include working directly with clients to ensure their accommodation is safe, assisting with personal finances, benefit applications, and budgeting. Additionally, you will maintain accurate records of client interactions and support data reporting for the Service and Regional Managers.
Key Responsibilities
1. Manage referrals and client outcomes using our case management system and the Bristol Housing Support Register (HSR).
2. Conduct needs and risk assessments to create personalized support plans.
3. Ensure clients understand their rights, responsibilities, and housing benefit claims.
4. Provide high-quality, person-centered support with a trauma-informed approach.
5. Maintain health and safety standards in client accommodation.
What We're Looking For
* A valid UK driving license and access to a vehicle with business insurance.
* An understanding of homelessness, social exclusion, and effective client-focused approaches.
* Empathy, resilience, and awareness of challenges faced by individuals with complex needs.
* Experience supporting vulnerable or marginalized groups.
* On-call availability: 1 in every 4 weeks (£105/week).
