We serve a population of nearly 8000 patients from a diverse population. To further strengthen our exceptional team, we are seeking an experienced and motivated pharmacist to join us.
The successful candidate will work within their professional boundaries, collaboration closely with out general practice team. They will receive ongoing support, guidance and mentorship from our GP Partners to ensure professional development and integration into our multi-disciplinary team.
Main duties of the job
To work as part of our practice clinical team to:
* conduct comprehensive clinical medication reviews, proactively managing patients with complex polypharmacy.
* provide expert advice on medicines and prescribing to general practice staff.
* support the repeat prescription process, manage acute prescription requests and oversee medicines reconciliation during care transitions.
* promote safe prescribing practices and contribute to the development of systems that enhance medication safety.
* assist in achieving Quality and Outcomes Framework (QOF) indicators and participate in safety audits where appropriate.

About us
We are expanding our Clinical team at Brotton Surgery to meet the increase in our patient population.
We currently have 4 GP partners and 1 Management Partner. We have excellent links with the PCN and have access to various ARRS roles including other pharmacists who support our clinical work.
We encourage our staff to work with us to develop excellent care for our patients and will regularly take on board suggestions made and adapt practices. With regular team meetings to look at clinical areas as a team and learn from each other. We also offer excellent management and administrative support.
As we are a training practice, up‑skilling our staff is high on our agenda.

Medicine information to practice staff and patients
* Respond to medicine related enquiries from GPs, other practice staff, other healthcare teams (e.g. community pharmacy) and patients/carers with queries about medicines.
* Suggesting and recommending solutions.
* Providing follow up for patients to monitor the effect of any changes.
Repeat prescribing
* Support safe and timely repeat prescribing, repeat dispensing, electronic prescribing by completing medication reviews. Ensure patients have appropriate monitoring tests in place when required.
* Identification of cohorts of patients at high risk of harm from medicines through pre-prepared practice computer searches. This might include risks that are patient related, medicine related, or both.
Information management:
* Analyse, interpret and present medicines data to highlight issues and risks to support decision making.
* Implementation of local/national guidelines and formulary recommendations
* Support the development and implementation of medicines‑related procedures within the practice, including appropriate acceptance of amber (shared care) drugs, medicines monitoring and recalls.
* Support the agreement and review of Prescribing Formularies and Protocols and monitor & address compliance/adherence levels.
* Support the delivery of QOF, incentive schemes, QIPP and other quality or cost effectiveness initiatives.
Education and Training:
* Provide education and training to primary healthcare team on therapeutics and medicines optimisation.
Care Quality Commission
* Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved.
Public health
* To support public health (including vaccination) campaigns. To provide specialist knowledge on all public health programmes available to the general public.
Person Specification
Qualifications
* Must be registered as a pharmacist with the General Pharmaceutical Council.
* Hold or be working towards an independent prescribing qualification
* Demonstrates an understanding of, and conforms to, relevant standards of practice.
* Follows professional and organisational policies/procedures relating to performance management.
* Membership of Primary Care Pharmacy Association (PCPA).
* Membership of the Royal Pharmaceutical Society.
Experience
* Experience and awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice.
* Demonstrates ability to integrate general practice with community and hospital pharmacy teams community groups.
* In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
* Experience of working in primary care
* Experience of working in a GP practice
