Main duties of the job
We are seeking a dynamic and compassionate Clinical Lead Community Nurse to spearhead our nurse-led service. This role is ideal for a registered nurse with a passion for community care, clinical leadership, and staff development. You’ll be instrumental in triaging care concerns, supporting care teams, and ensuring our clients receive timely, responsive, and high-quality care at home.
Key Responsibilities
* Lead the delivery of a nurse-led community care model, ensuring clinical excellence and continuity of care
* Triage care concerns raised by staff, families, or clients, and implement timely interventions to prevent deterioration
* Provide clinical training, mentorship, and supervision to care staff, building their confidence and competence
* Develop and maintain clinical protocols and pathways to support proactive care planning
* Collaborate with GPs, district nurses, and other health professionals to ensure integrated care delivery
* Monitor outcomes and contribute to reducing hospital admissions through early intervention and preventative care
* Champion a culture of clinical accountability, safety, and compassion across the service
* Participate in audits, quality improvement initiatives, and service development

Role Summary

Key Responsibilities

What We’re Looking For
* Registered Nurse (Adult or Mental Health) with valid NMC registration

Person Specification
Qualifications Essential
* Registered Nurse with current NMC registration
* Minimum 3 years post-registration experience in community or primary care
* Proven experience in clinical leadership, care coordination, or working with multidisciplinary teams
* Strong clinical assessment and decision-making skills
* Knowledge of safeguarding, risk management, and CQC standards
* Excellent communication, coaching, and interpersonal skills
* Ability to develop care plans and clinical protocols
