To liaise with all members of the clinical team, patients, and other healthcare professionals to provide an accurate, timely, and qualitative process for prescription issues and medication updates for patients registered at Montpelier Centre. The post holder will be expected to deal with queries from patients, local pharmacies, hospital departments, nursing homes, and GPs, as well as the Clinical Pharmacist daily.
Main duties of the job
Liase with all members of the practice team, particularly with clinical staff, to ensure that patients' prescription requests are responded to within the agreed timescales.
Coordinate with relevant pharmacies regarding queries about patients' prescriptions.
Collaborate with other members of the medicines management team when additional support is needed for medication changes, dossettes, or documents, to agree on workload delegation. This can be discussed with the line manager or Practice Manager (PM) if necessary.
Ensure all prescription coordinator duties are covered during absences, in conjunction with the line manager/PM.
Monitor changes to medication, including overuse and underuse, and alert the clinician accordingly.
Use judgment to determine whether medication should not be issued on a repeat.
Generate patient requests for prescription items not listed by liaising with the relevant clinical team member.
Handle discharge summaries and other clinical documents related to medication changes or new medications within the EMIS workflow (once coded) on designated days.
Update medication changes on patients' medication screens within EMIS.
Forward documents to GPs where necessary.
Take responsibility for the compilation and updating of dosette boxes.

* Experience working in an integrated multi-skilled team.
* Experience working in a pressurized environment.
* Experience using EMIS Web software.
* Experience in customer service.
* Extensive knowledge of prescription management.
