Main duties of the job
This is a dynamic, patient-facing Clinical Pharmacist role, focused on delivering evidence-based pharmaceutical care in Primary Care. You will use your expert knowledge of medicines to support patients with specific disease areas and long-term conditions.
Key responsibilities include:
* Clinically assessing, diagnosing, and prescribing (where qualified)
* Optimising medicines and improving prescribing safety
* Collaborating with a multidisciplinary team to enhance patient outcomes
* Supporting medication reviews and structured medicines optimisation
* Contributing to service development and innovative working practices
* Working with Divisional Boards and the National Modality Business Support Team to deliver strategic priorities

Qualifications
* Registered with the General Pharmaceutical Council (GPhC).
* Postgraduate Certificate (PgCert) or equivalent.
Experience
* Experience working within General Practice or Primary Care.
* Relevant experience as a Pharmacist in a healthcare setting.
Knowledge
* Strong understanding of medicines optimisation.
* Excellent mathematical and scientific knowledge.
* Thorough understanding of prescription drug legislation and regulations.
* Knowledge of long-term condition management.
* Understanding of clinical governance in Primary Care.
* Awareness of local and national healthcare priorities.
* Mandatory vaccination evidence is required for this role, including Hepatitis B x3, titre levels and or booster, MMR, and Varicella.
