Main duties of the job

A successful Health Care Assistant will have:

* Previous experience in a caring role
* A recognised caring qualification

Job responsibilities

Post: Health Care Assistant

Job Purpose:

Under the direction andsupervision of qualified registered staff, to maintain and support the highest standards of nursingcare, so as to ensure that a high quality and comprehensive service is provided to all patients, their families and carers.

Responsibilities:

1.0 Clinical

1.1To ensurestatutory obligations are met that are relevant to the clinicalarea.

1.2To ensure quality standards are maintained in order to providethe highest standards of holistic care.

1.3To maintain and support the provision of assessment,planning, implementation, and evaluationof nursing care, within the limits of your qualifications and under the supervision of qualified staff.

1.4Work to the agreed models of care and work allocation.

1.5Escalate any clinical or non-clinical incidents to thenurse in charge, and complete incidentdocumentation via Sentinel. Assist withinvestigations and risk assessments as appropriate.

2.0 Professional

2.1 Tounderstand and work within the Health and Social Care Act 2008, and PT businessplan and philosophy.

2.2Work as part of the multidisciplinaryteam, providing holistic care to patients, their families and carers.

2.3 Delivera safe and high standard of care in accordance with Phyllis Tuckwell policies, guidelines.

2.4 Where appropriate and under the supervisionof qualified staff, provide information andsupport to patients, carersand relatives.

2.5 Maintain effective relationships with colleagues.

2.6 To maintain accurate, legible andcontemporaneous records on all patients, to ensuredocumentation meets agreed PT standards and adheres to Caldicott Guidelines.

3.0 Managerial

3.1 Towork in accordance with the PT, Health and Safety Governance policies.

3.2To effectively maintain and support theclinical area and monitor resources including clinical supplies and equipment.

3.3 Toensure a team environment is maintained by developing effective relationshipsand being considerate towards colleagues, to create a team spirit and providepersonal support.

3.4 Manage your own time effectively to meet PTaims and objectives.

4.0 Training and Development

Team

4.1Maintainand support the professional development and safe practice of others throughpeer support, leadership, supervision and teaching.

Personal

4.2Maintainup-to-date knowledge of PT guidelines, policies and procedures.

4.3To identify and develop own knowledge andclinical skills, through continual professionaldevelopment, clinical supervision, discussion with other members of the MDT,and through PT Annual Appraisal process.

5.0 Research

5.1 Participatein research and audit within the PT environment.

5.2 Assist in implementing the findings ofevidence-based practice.

6.0 Safeguarding, Equality, Diversityand Inclusion

Role holders are required tounderstand their responsibilities in the safeguarding context, ensuring thatPhyllis Tuckwell reduces the risk of harm or abuse of adults and children atrisk.

Youare required to:

* adhere to Phyllis Tuckwells Safeguarding Policy(held on the NEST), which supports the local authoritys multiagency safeguarding policy, the law (Care Act 2014) and Mental Capacity Act2005 (including Deprivation of Liberty Safeguards).
* inform the appropriate PT professional wherethere is concern that an adult or child may be at risk of harm, abuse, orneglect.

Person Specification


Experience

* Evidence of care skills from working in a care home, patients own home, or hospital setting.
* Understanding and experience of caring for dying patients with progressive diseases.


Computer Skills

* Be in possession of basic IT skills e-mail and internet usage and a willingness to increase IT skills.


Qualifications

* Care Certificate /willing to undertake.
* Evidence of undertaking health care courses or other life skills.
* Has NVQ / QCF / Regulated Qualifications Framework (RQF) / City & Guilds level 2 or above.


Quality

* Commitment to providing high quality, individualised holistic care to patients and their relatives / carers.
* Demonstrates an understanding of the importance of Health and Safety and clinical governance.
* Demonstrates an awareness of patient confidentiality and the principles of information governance.
* Demonstrates an understanding of the importance of PT policies and procedures.


Relationships and Communication

* Good communication and interpersonal skills.
* Empathy and compassion.
* Demonstrates a caring approach.
* Ability to write in a clear and legible form.
* Understand the importance of reporting any changes in a patients care to colleagues.
