Main duties of the job
This job is including face2face to discuss medication issues with patients as needed and appropriate, carry out medication reviews and chronic disease medicines management. Prescribing medication following prescribing systems and policies. Liaison with community and hospital pharmacies. Medication reconciliation and medicines information according to guideline.

Due to retirement an opportunity has arisen to join our friendly forward thinking practice. We are situated in North East Wales on the Welsh/English border. There are good connections to nearby cities with the historical city of Chester being only 5 miles away. There are also excellent schooling options in the local area. We deliver our GMS contract over 3 sites. Our main site is Hawarden Health Centre with branch sites in Saltney and Buckley. Hawarden is located 6.7 miles from Chester and is 4 miles from the English border, we are close to motorway links giving direct access to Liverpool and Manchester
Job responsibilities
The Practice Pharmacist prescriber is an integral part of the clinical team, using and sharing pharmacy prescriber expertise to support the GPs and the whole practice in effective medicines management and optimal patient care. To develop the role of Practice Pharmacist prescriber in close collaboration with the partners.
Person Specification
Experience
* Have relevant clinical experience
* Experience of working in GP surgery
Qualifications
* Must be registered with the General Pharmaceutical Council (GPhC), have relevant clinical experience and complete an accredited independent prescribing course.
