We are looking for a passionate and experienced Social Worker to join our Keyworker Service within the Birth to Settled Adulthood (B2SA) team. This is a unique opportunity to support children and young people aged 0‑25 with autism and/or learning disabilities who are at risk of mental health hospital admission or are currently in inpatient settings.
What You’ll Be Doing
* Supporting children and young people on the Dynamic Support Register (DSR).
* Acting as a consistent point of contact for families and professionals.
* Developing personalised support plans and facilitating Care, Education and Treatment Reviews (CETRs).
* Working collaboratively across services to prevent hospital admissions and support timely discharges.
* Contributing to service evaluation and continuous improvement.
About You
* Is a qualified and registered Social Worker.
* Has experience working with children and young people with complex needs.
* Can build strong, trusting relationships with families and professionals.
* Is proactive, organised, and solution-focused.
* Has the ability to travel across Dorset (a suitable vehicle is essential).

Additional Information
