Ackerman Pierce is currently supporting a leading local authority within Windsor & Maidenhead to recruit for a Senior Social Worker in the Hospital Discharge Team within the Social Care industry. This will be on a permanent basis paying £45,831 - £50,414 per annum plus £3,000 annual retention bonus.
Responsibilities include:
Working with Vulnerable Adults within a hospital setting
Report writing and making assessments
Developing care plans
Care management
Managing complex cases
Liaising with other professional bodies
Arranging safe and timely discharges for service users
Self-directed support / personal budgets
Supervise Social Workers and Case Management Officers
Handle complex cases, including MCA Assessments, Court of Protection applications, and s42 safeguarding enquiries
Experience preferred:
Post qualifying experience of working as a Senior Social Worker - Hospital Discharge team within the UK Social Care industry.
Minimum Requirements:
HCPC Registration
1 year post qualifying experience within UK Social Care industry
Enhanced DBS/CRB (we can obtain for you)
