Salary: Starting Salary is £28,598 (Level 1) rising to £31,022 (Level 4) per annum (pro-rata for part-time)

About the Role

Health and Social Care Officers (HSCOs) are responsible for triaging, co-ordinating referrals and providing information and advice to citizens and professionals at our busy 'front door' for Nottingham City Adult Social Care. You will enable decision-making and risk management on behalf of citizens requesting access to adult social care support.

HSCOs assist in the delivery of an efficient and effective referral service for vulnerable adults, sometimes with complex needs or in difficult and unpredictable situations. Applicants should have a good understanding of the variety of services that are available to meet an individual's needs whether through preventative support or commissioned services.

Essential Requirements- What skills are we looking for?

* Communication Skills- Excellent verbal and written communication with the ability to ask sensitive and appropriate questions; using active listening and empathy when dealing with vulnerable individuals.
* Knowledge and Understanding- A good understanding of adult social care issues, adult social care legislation (e.g., Care Act 2014) and an awareness of safeguarding principles and procedures, alongside familiarity with local services and referral pathways.
* Analytical and Decision-Making skills- Ability to assess urgency and risk based on limited information, showing a confidence in escalating concerns appropriately and using sound judgement in triaging referrals to the correct service or team.
* Technical Skills- Proficiency in using case management systems and telephony software with the ability to accurately record data and navigate multiple systems simultaneously.
* Interpersonal Skills- Ability to remain calm and composed under pressure, maintaining a respectful and non‑judgmental approach while effectively managing conflict and navigating challenging situations with professionalism.

A DBS enhanced check for a regulated activity is required for this post.
