We have an exciting opportunity for a Band 5 enthusiastic, motivated, and compassionate nurse to join our established multidisciplinary Care Community Team to support and deliver a range of quality nursing care to our community patients.

Your skills will be really valued by our teams and patients alike. If you are new to community health, this is a great opportunity to gain experience in this unique field of nursing.

We will do our best to welcome you and help you to settle in as a member of the team. Your contribution, time, skills and caring will make a huge difference to the patients we care for.

This post is Full time - 12 months fixed term.

Shortlisting for this vacancy is due to commence on 23/02/2026 and the proposed interview date is 04/03/2026.

Main duties of the job

Our care community team is an 8am-8pm service based in Princeway Health Centre, covering Frodsham and surrounding geographic area. It's important that you can drive and have access to your own vehicle for work. But of course, mileage expenses are reimbursed in addition to your hourly rate.

As a Community Staff Nurse the successful candidate will undertake duties between the working hours involving the delivery of high-quality nursing services and the holistic assessments of patients ensuring a person-centred approach.

You will work alongside various professionals to provide quality and seamless care to patients predominantly in their own home. You will be proactive in coordinating their care needs as appropriate, avoiding unnecessary admission to hospital or long-term care settings.
