Overview
Barrow Town District Nurses have an exciting opportunity to recruit a permanent Band 6 District Nursing Sister/Charge Nurse. The successful candidate will ideally have at least one year's experience in Community Nursing and have completed or be working towards a relevant post registration qualification. You will be working within the Furness district as part of a dynamic and supportive team who continuously strive to deliver gold standard patient care within our community.
Responsibilities
* Provide holistic, patient‑centred care delivery while facilitating and undertaking personal and professional development, supported by the team and Community Care Group.
* Work across the Lancaster area and other community nursing teams as part of the Integrated Care Community (ICC), covering a 24‑hour period including weekends and bank holidays, 7 days a week.
* May be required to work across a variety of District Nursing teams in the Community Care Group to maintain service delivery.
* Work with health and social care disciplines to prevent hospital admission, facilitate hospital discharge, and promote independence through reablement and management of long‑term conditions.
* Undertake complex nursing assessments and develop and deliver programmes of care with patients, families and carers.
* Utilise specialist skills and knowledge with a genuine passion for high‑quality community care.
* Support staff health and wellbeing within the team.
* Meet the needs of the role across the Furness district and associated community facilities as required.

* Posts available: 37.5 hours
* You will need a full UK driving licence and access to a vehicle to fulfil this role.
* We operate from three main hospitals – Furness General Hospital (FGH) in Barrow, the Royal Lancaster Infirmary (RLI), and Westmorland General Hospital (WGH) in Kendal, plus community premises including Millom Hospital, GP Practices, Queen Victoria Hospital in Morecambe, and Ulverston Community Health Centre.
* FGH and RLI provide a range of General Hospital services with Emergency Departments, Critical/Coronary Care units and Consultant‑led services. WGH provides General Hospital services and an Urgent Treatment Centre for non‑life‑threatening conditions. A range of planned care and local outreach services are also provided from community facilities across Morecambe Bay.
* Community services for adults are delivered in people’s homes, community centres, clinics, GP Practices, community hospitals and our main hospitals. The aim is to help people remain independent and improve health by managing conditions through high quality care, advice and support.
