We are looking to secure a registered Band 6 Nurse to join our established Community Nursing Team to deliver excellent health care across St Helens.
Job Overview
Community Grade NHS AfC: Band 6, Permanent, Full time – 37.5 hours per week. Site: St Helens Borough Town. Salary: £39,959 – £48,117 per annum.
Responsibilities
* Provide skilled nursing care and advice to patients in their own homes and residential or other settings, undertaking comprehensive specialist assessments.
* Promote healthy lifestyle practices to the public through teaching.
* Manage clinical caseloads efficiently and effectively.
* Prioritise health problems and intervene appropriately in complex and urgent situations.
* Contribute to quality improvement via clinical governance, supervision and care excellence.
* Maintain clear, concise, legible, contemporaneous patient records in line with Trust policies and NMC standards.
* Liaise with other health and social care professionals to ensure holistic patient care.
* Ensure confidentiality of patient information and adhere to data protection regulations.
Qualifications
* Registered Nurse with current NMC registration.
* Evidence of postgraduate study in a specialist area of practice.
* Postgraduate qualification relevant to specialist area of practice.
Skills & Experience
* Excellent communication, negotiation, and listening skills.
* Ability to delegate, coordinate, and manage difficult situations.
* Proficiency in assessment tools and evidence‑based practice.
* Strong organisational and time‑management abilities.
* Computer literate and adept in relevant IT systems.

* Understand own scope of practice and its implications.
* Demonstrated leadership, management, and mentoring experience.

Essential Condition
An enhanced DBS with barred lists for both Adults & Children is required for this post.
