As a Social Worker, you will be a member of the multi-disciplinary team, providing high-quality social care services to adults of all ages experiencing mental ill-health. The core functions of the role include:
1. Providing assessment, care planning, and coordination of care from a social perspective for adults accessing the service.
2. Delivering recovery-focused care and support to service users and their carers, emphasizing principles of choice, control, independence, and safety.
3. Acting as a champion of social care and safeguarding, with an understanding of relevant legislation and statutory guidance.
4. Ensuring compliance with child and adult safeguarding policies, including follow-up on safeguarding referrals and child visit requests.
5. Liaising with families, carers, and nursing staff to arrange visits and foster good relationships.
6. Involving service-users and carers in service improvements based on their experiences.
7. Negotiating and liaising with other agencies such as the Ministry of Justice, Police, Children's Services, and Carers' Networks.
8. Supporting colleagues in the appropriate use of the Mental Capacity Act.
9. Maintaining accurate and up-to-date records of all work undertaken.
10. Championing equality and diversity and challenging discrimination in all its forms.
