We are looking for apassionate and committed individual to join the Continence and Urology Servicehosted within our Neighbourhood Integrated Teams across Barnsley.
We have an excitingopportunity for an enthusiastic, motivated, HCA to work within the Continenceand Urology Service throughout the Barnsley area.
The use of a vehicle toundertake any necessary travel in connection with the duties of the post isessential.
Atthe time of advertising, this role does not meet the minimum requirements setby UK Visas and Immigration to sponsor candidates to work in the UK. For thisreason, we are unable to sponsor anyone for a skilled worker visa for this role
Weare happy to accept applications from candidates who can prove their right towork in the UK or via alternative visa routes. Work with a degree of independence within defined parameters, as directedby a registered health care professional (or associate practitioner if part ofthe team).
Refer and seek guidance/supervision on issues outside of postholders level of competency or authority to more senior health careprofessionals.
Clearly, accurately and contemporaneously, contribute to care documents,report observations and other service user information, in written andelectronic format. Demonstrate competent IT skills, accurate data inputting anduse of the internet.
Accurately record clinical measurements and physical observations, whichmay include: monitoring of blood pressure, pulse, temperature, respiration,weight etc, and handle specimens of bodily fluids. Identify potential risks andrespond appropriately.
As part of a team, deliver person centred care whilst meeting theservice standards and objectives
Work in partnership with individuals, formal and informal carers andother services to promote the concept of self- and patient led care wherepossible, providing appropriate education and support to maximise theindividuals independence and understanding of their condition.

Under thesupervision/direction of registered health care professionals/associatepractitioner, within a delegated framework for agreed tasks, seek support asand when necessary, in the delivery of physical care to a defined group ofpeople.
Have the flexibility and skill set, with appropriatetraining, to work across planned and crisis pathways supporting patients toremain well in their own homes, or long-term place of residence and / orprevent hospital admission
As part of a healthcareteam, provide the highest standards of safe and compassionate planned care topeople that maximises their recovery and rehabilitation potential.
Demonstrate anunderstanding of the care and treatment programmes of a designated group ofservice users to enable you to instruct, advise & guide service users,family and carers. To make the appropriate adjustments independently within ownlevel of competence and be able to communicate any changes to service users andregistered health care professionals.
Be required to concentratefor periods of time, depending upon the care intervention/care plan eg,following the observation policy, taking, recording of physical observationsetc, and responding to issues of concern appropriately. This may include areasof work requiring physical effort, with regard to risk, health & safety.
Understand the principles underlying care planning eg,enabling the service user to reach an optimum level of functioning/independence/recovery.
Implement care withinwell-established policies, procedures, protocols, guidelines and establishedclinical care plans. Organise a range of activities e.g., Diabetes, Obesity, smoking cessation etc, providingadvice and support.
Undertake the physicalaspects of care e.g., Use items ofequipment in delivering care e.g., Attend to the serviceusers personal care needs as required
Participate in a flexiblerostering arrangement providing 24-hour/7-day care (as appropriate to the role)
Work in an integrated andpartnership way with primary, secondary, social care, the independent andvoluntary sector and others to improve the health and care of individuals,families and communities, particularly the most vulnerable;

GCSE A-D or grade 3-9 in Maths and English or equivalent.
Apprenticeship level 3 or equivalent eg, RQF(regulated Qualification Framework)NVQ level 3 in Health & Social Care or equivalent.
Or, level 2 qualification with the ability to demonstrate competencies at level 3.
Care Certificate or required to achieve within 12 weeks of commencement.
Level 4 units in Health & Social Care.
A current driving licence and access to a car during the working day is essential (reasonable adjustments will be considered for any applicants who are unable to drive due to a disability).

A satisfactory sickness record over previous 2 years (subject to the need to act with fairness and equality of opportunity, particularly where sickness is related to a disability and/or pregnancy).
