Typical day as a Support Coordinator
Develop personalised support plans, coordinate Support Workers and review progress
Work with Newcastle services to reduce homelessness, supporting vulnerable women with complex needs
Provide high quality housing (24-bed and dispersed properties) where customers feel secure
Connect customers with specialist services and help them achieve personal goals
Support daily life skills, benefits, bills, tenancies and moves to independence
Build trust, manage challenges and collaborate closely with colleagues

You bring
Passion to support our customers to live their best life, working collaboratively with an eye for detail
Experience of creating person-centred support plans and supporting colleagues to deliver them
Experience of coordinating and assessing customer referrals
The ability to workon your own initiative, remaincalm under pressure and havea resilient approach

Our team
Youll join a passionate team of 19, who support customers to achieve their goals and celebrate every success, while looking out for each other too. Our Manager Tracy has led Cedar House for 14 years, supported by Fintan, our Senior Client Services Manager with over 25 years at Home Group, and Alex our Mental Health Practice Lead, who is always on hand with expert advice.
Job details
Youll work on a rota basis between 7.00am to 8:30pm
Includes days, evenings, weekend and bank holidays
Occasionally our customers' needs mean your start and finish times may change. Where we ask you to be flexible, we will always try to reciprocate
Confident using technology for plans, online learning and teamwork
Youll need an Enhanced DBS check done and we pay for that
