We have an exciting opportunity for a hard working and enthusiastic professional to join our Intermediate Care Team in Blantyre LIFE
In this role, you will be working with people discharged from a hospital setting who require further a period of reablement, recovery, time and assessment to determine any needs, this may involve home visits to support discharge.
In order to be successful in this role, you will have the ability to gather, select and analyse information to support your assessment decisions and you will be able to identify the underlying cause of any functional difficulty.
This will require you to provide equipment and/or adaptations as a direct solution to the need or refer onto other statutory or voluntary services.
You should have the ability to work autonomously and to tight timescales and be able demonstrate responsibility for your own learning needs.
You should have an HNC in Occupational Therapy Support or be willing to work towards gaining this qualification.
Supervision and peer support will be provided regularly
