Job Overview
This is an exciting opportunity to join our developing Pulmonary Rehabilitation team within Hywel Dda University Health Board. The Pulmonary Rehabilitation team is part of the Multi-Model Rehabilitation service. This part-time role involves 22.5 hours per week (Monday, Tuesday, and Thursday). The service operates across the University Health Board, covering Pembrokeshire, Ceredigion, and Carmarthenshire. The post is based at Glangwili Hospital, with potential to deliver services across Hywel Dda.
Pulmonary rehabilitation includes community-based exercise classes, education, assessments, and home visits. We collaborate with the national exercise referral scheme and aim to develop links with third-sector activity providers to support service users post-rehabilitation.
The successful candidate will join a friendly, proactive team, working collaboratively within a community-based multidisciplinary team. The service promotes staff growth and service development.
You will provide high-quality, evidence-based, person-centered care to patients across Hywel Dda as part of the Pulmonary Rehabilitation team.
Main duties of the job
1. Form part of an existing team of physiotherapists responsible for delivering patient rehabilitation services within their clinical specialty.
2. Collaborate with multidisciplinary teams to deliver holistic care.
3. Manage your own caseload, including self-referrals and referrals from GPs, AHPs, social services, and consultants.
4. Operate autonomously with high clinical and technical skills, using evidence-based practices.
5. Participate in professional development, including supervising junior staff and students.
6. Support service modernization and development, including audit and evaluation.
7. Participate in a flexible 7-day service rota and on-call respiratory service as required.
