We’re currently looking for Clinical Site Coordinator Nurses (Band 6 equivalent) to join our highly skilled team of health care professionals. In this role you will provide leadership and management of the care team to ensure effective delivery of care for children and young people with life‑limiting and life‑threatening conditions, whether they are with us for a short break, emergency care, symptom control or end of life across both Naomi House & Jacksplace.
Key Responsibilities:
* Lead and manage the care team, ensuring high‑quality care and support for young people.
* Coordinate care activities across acute, emergency, and end‑of‑life settings.
* Collaborate with multidisciplinary teams and external agencies to deliver comprehensive care.
* Implement and monitor quality improvement initiatives.
* Ensure compliance with safeguarding and clinical governance policies.
