Essential Criteria

* A degree in Psychology, Occupational Therapy, Nursing, NVQ Level 3 in Health and Social Care or related field or, equivalent work experience
* Proficiency in performing personal care tasks, ensuring the well-being and comfort of the individual under your care.
* A naturally positive attitude that fosters a supportive and uplifting environment for those you work with.
* Proven ability to inspire and motivate the individual you support, empowering them to pursue their goals and aspirations with determination and enthusiasm.
* Proven ability to lead and/or manage a team.
* Full UK driving licence and access to a vehicle.

We are currently recruiting 2 Case Managers to join our team of operational managers;


Role 1

Experience supporting individuals with complex nursing needs, either within healthcare settings or in their own homes, is essential for this role and your caseload. Due to the personal care needs involved, this role is open to female applicants only, in accordance with Schedule 9, Part 1 of the Equality Act 2010.

Shift pattern(s) available

This is the perfect role for someone who would like to expand their Nursing experience whilst gaining or expanding their current management experience. If you are a senior support worker or team lead, this could be the perfect career development for you. We are looking for someone passionate about providing quality service and ensuring that the people we support and our support workers reach their full potential. This is an exciting opportunity to join our dedicated North East service as we continue our growth across the region.


Role Specifics


Role 2

Experience in providing support to individuals with Brain Injuries, following Rehabilitation care plans and understanding their unique needs, is essential for this role and your caseload. Due to personal care needs, this role is open to male applicants only, in accordance with Schedule 9, Part 1 of the Equality Act 2010.

Shift pattern(s) available

This role is ideal for someone wanting to expand their ABI and Rehabilitation experience while gaining or enhancing management skills. We seek a resilient Case Manager capable of navigating complex situations confidently and professionally, managing conflicts effectively with a solution-focused approach. Strong communication skills are essential to handle difficult conversations positively, ensuring all parties feel heard and valued, with the best outcomes for service users. If you are a senior support worker or team lead, this could be a great career move for you.


Additional Tasks for Both Roles


Support Provision

* Your working hours will include delivering support in clients’ homes and office-based work on a rota over seven days.
* Deliver high-quality, safe, and person-centered support.
* Participate in contingency plans for absences and holiday cover.
* Safeguard the individuals at all times, acting in their best interests.
* Develop rapport and professional relationships with the individuals supported and their next of kin.
* Identify support and personal goals with the individuals.
* Manage and lead Support Workers to follow and implement care plans.
* Coordinate support sessions with stakeholders, ensuring funded hours are adhered to.
* Participate in an on-call rota, providing advice and support outside regular hours.
* Keep the Service Manager and Operations Manager informed about support delivery and issues.


Line Management

* Act as line manager to Support Workers.
* Maintain rota management for the support team.
* Conduct regular supervisions and observations.
* Support, guide, and train staff as needed.
* Lead by example and communicate effectively.
* Assist in recruiting Support Workers based on needs and preferences.
* Brief and train Support Workers on individual needs.
