LANGUAGE REQUIREMENTS

* Listening and Speaking - Intermediate Level: able to maintain fluent conversation on everyday subjects and discuss cases in the field of work. Able to follow a discussion in Clear Welsh on familiar matters relating to the post. Able to contribute and answer questions.
* Reading and Understanding - Intermediate Level: understand everyday correspondence on familiar issues at work; understand longer reports in Clear Welsh and identify main points. Support may be required with vocabulary.
* Writing - Intermediate Level: able to write letters for specific purposes, e-mails and short reports in Welsh and English using familiar vocabulary and phrases; these will be checked before being sent.


Job Description

Purpose of the post

* Ensure that the people of Gwynedd are at the heart of everything we do.
* Help individuals receiving support to live their lives in their own homes as they wish.
* Encourage and support individuals to develop their skills to live successfully within their community.
* Ability to set objectives, solve problems, and recognise changes in circumstances and needs.

Responsibilities

* Ensure appropriate use of individuals' property and specialist equipment, reporting defects and following practice and training protocols.
* Build strong relationships with people to understand what would make a difference and empower the individual's voice.
* Contribute to, and follow, personal care plans created in partnership with the individual; update or review plans as needed in collaboration with the Community Care Co-ordinator or other professionals.
* Contribute to assessment, monitoring, planning, and implementation of activities per individual needs, including participation in programmes, exercises, and interventions.
* Be innovative in providing care that includes people, promoting rights, and encouraging informed risk-taking.
* Know what is happening in communities and seek solutions to help people live as fully as possible using community resources and activities.
* Test new ways of supporting and think creatively, drawing on the individual’s strengths and informal networks.
* Participate in multidisciplinary conversations and contribute to writing about personal well-being outcomes; support individuals to achieve results using available resources.
* Work with others to identify obstacles to achieving results and discuss with the wider team to implement changes and learn from experience.
* Follow the care plan and use specialist equipment following movement and handling plans; report problems, defects, and obstacles promptly.
* Have awareness of simple technology to help independent living; recognise the need for support equipment such as Telecare and assist users in using it.
* Undertake simple health tasks (e.g., administering eye and ear drops, changing catheter and stoma bags) and report concerns to the Community Health Care Team as required.
* Carry out accurate administrative tasks relevant to the role (recording and documenting information in service user files daily).
* Comply with Cyngor Gwynedd's Medicines Code of Practice and undertake detailed assessments when arriving at each call, considering health and well-being, and respond quickly to changes.
* Make informed decisions independently when appropriate, seeking support from the wider team as needed.
* Work as part of a team to solve problems; support new staff in their development using knowledge and experience.
* Follow an ethos of enabling and promoting independence for individuals.

Communication and Relationships

* Work in partnership with colleagues in health and care, including social workers, community nurses, occupational therapists, physiotherapists, GPs, and third-sector partners as appropriate.
* Act to build lives linked to family and community networks and be clear about what you are there to help the person achieve.

Training and Supervision

* Attend mandatory training and other identified relevant training.
* Complete the QCF Level 2 qualification; engage in continuous professional development.
* Participate in supervision and team meetings to support service and staff well-being.
* Register with Social Care Wales and ensure required training hours are completed annually; adhere to the professional code of practice for social care.

Safeguarding

* Report any concerns or suspicions of abuse of a child or vulnerable adult.

Policies

* Respect confidentiality and comply with information management standards and Data Protection legislation.
* Comply with Health and Safety rules and the Council's policies on equal opportunities and other guidelines.

Other

* Commitment to reducing the Council’s carbon emissions and to encouraging others to do the same.
* Undertake other reasonable duties appropriate to the salary and responsibility level. Be prepared to work across other sites as needed and to work unsocial hours on a rota basis (including weekends and bank holidays).
