This is a great opportunity to build skills in hands‑on patient care while supporting people to regain confidence and independence. You’ll work as part of a friendly multidisciplinary team, helping patients in the Rehabilitation and Intermediate Care Service. Your support will make everyday activities safer and easier for people recovering from illness or injury. You’ll follow individual care plans and help create a calm, respectful environment where patients feel encouraged and understood. What you’ll do: • Provide personal and clinical care in line with each patient’s care plan • Record observations such as blood pressure, pulse and blood glucose • Support safe moving and handling and help patients with daily activities • Keep patient information accurate on the Electronic Patient Record (EPR) • Prepare equipment, maintain stock levels and assist with basic admin tasks What we’re looking for: You’ll bring kindness, patience and good communication skills. You might have worked in health, homecare or social care before, or gained caring experience elsewhere. You’ll need a Level 2 qualification (or equivalent experience) in Adult Health and Social Care and be willing to complete the Care Certificate.
