Transition Keyworker
Knowsley Community College is seeking a dynamic and motivated Transition Keyworker to support foundation learning students and enable successful transitions from schools and other post‑16 provision into college.
Key Responsibilities
* Ensure students with an EHCP make a successful transition into college.
* Work closely with other agencies, institutions and parents to design a transition plan for supported learning students.
* Provide a fully supported transition into college, including follow‑up support where necessary.
Required Qualifications
* Level 3 qualification in a relevant subject area.
* SEND qualification.
* Mathematics and English to Level 2, or willingness to work towards Level 2.
* Experience of working with young people in an advisory or support role in FE.
* Full understanding of SEND Code of Practice and transition responsibilities.
* Understanding of the special educational needs, disabilities and other barriers that young people may present within school.
* Ability to profile and assess needs, devise a transition plan and contribute ideas for appropriate support from college.
* Ability to build rapport with young people and offer unbiased advice to support their next steps in education.
* Knowledge and understanding of EHCPs and the role of colleges.
* Excellent written skills, including excellent spelling and grammar.

Salary: £12,774 to £14,568 per year.
