Primary Care Networks (PCNs) form a key building block of the NHS long-term plan. Bringing general practices together to work at scale has been a policy priority for some years for a range of reasons, including improving the ability of practices to recruit and retain staff; managing financial and estates pressures; providing a wider range of services to patients; and facilitating easier integration with the wider health and care system.
Following the release of ARRS GP funding, we are looking for newly qualified GPs (within 2 years) to join Chester Central Primary Care Network on a fixed-term salaried contract. We will consider applications for a minimum of four sessions with a start date as soon as possible.
As a reflection of the lower sessional rate, the post holder won't be required to complete home visits.
Our clinical system for all practices is EMIS Web, supported by a structured administrative team to assist with letters, results, medication management, etc.

Main duties of the job
1. Working across two or three of the six practices, undertaking duties including face-to-face and telephone consultations and queries, with no home visits.
2. Making professional, autonomous decisions regarding presenting medical problems, whether self-referred or referred by other healthcare workers within the organization.
3. Assessing healthcare needs of patients with undifferentiated and undiagnosed problems.
4. Screening patients for disease risk factors and early signs of illness.
5. Developing care plans for health in consultation with patients and in line with practice disease management protocols.
6. Recording clear consultation notes to agreed standards.
7. Collecting data for audit purposes and complying with QOF requirements.
8. Compiling and issuing computer-generated acute and repeat prescriptions.
9. Performing all general duties and responsibilities associated with a GP working within primary care.
About us
Chester Central PCN is a well-established PCN delivering excellent care to over 41,000 patients across Chester. It comprises six practices: four in one building (The Elms Medical Practice, Fountains Medical Practice, Northgate Medical Centre, Northgate Village Surgery), and Garden Lane Medical Centre and St. Werburghs Practice for the homeless within walking distance.
Our PCN includes additional roles such as social prescriber link workers, clinical pharmacists, pharmacy technicians, paramedics, dieticians, counselors, occupational therapists, and physiotherapists, which enhance patient services and allow doctors to focus on patient care.
We have two clinical directors working closely together and a PCN Manager overseeing daily operations, forming a strong leadership team.
The practices within the PCN have all secured a 'Good' rating from the Care Quality Commission. The PCN transitioned to an established limited company model in 2023.
