The Community Pathway Officer role will assist individuals experiencing mental health issues to stay in the community and prevent the need for a hospital admission or re admission.
The officer will provide an intensive (up to six weeks) outreach service to individuals who are in the crisis pathway, working in collaboration with the Norfolk & Suffolk Foundation Trust (NSFT), the service will support individuals to re-integrate into the community.
A full driving license and access to a vehicle are essential.
