What you will do:
Develop and implement care plans for young adults transitioning to independence.
Provide line management support, overseeing the performance and development of junior staff.
Apply knowledge of legislation including the Mental Capacity Act 2005 and the Care Act 2014.
Manage budgets effectively to support service users.
Liaise with professionals across health and social care to coordinate holistic services.
Apply safeguarding procedures to protect vulnerable adults and children.
Requirements:
Willingness to undertake line management responsibilities
Submission of a fully completed Adult MOC form
Valid driving licence
