Overview

The post holder will take responsibility under the direction of the Consultant body to deliver an expert nursing service to patients with Type 1 and Type 2 Diabetes. In addition, some of the caseload will include Maturity Onset Diabetes of the Young (MODY) and Cystic Fibrosis related diabetes, from diagnosis until discharge or transition to the adult service.

You will deliver specialist nursing skills, demonstrate sound clinical knowledge and expertise in the autonomous management of children and young people within the diabetes service, to include nurse-led consultations with the child and family.

You will ensure that patients and families are educated, informed and supported by providing clinical and social support in relation to their specific diagnosis. It will be necessary to work flexibly with a caseload of patients, across both the hospital and community settings.

The post holder will provide expert advice to other health care professionals and initiate guideline-based therapies to address acute management issues, you will establish clear lines of communication with primary healthcare teams in order to increase knowledge and facilitate the care of this group of patients in the community.

Working across organisational, regional and national settings and as part of the multidisciplinary team.


Main duties of the job

* To hold responsibility of the children's diabetes specialist nursing team
* It will be necessary to work flexibly across a caseload of patients across both the hospital and community setting for local patients and to provide expert advice to other health care professionals and initiate guidelines based therapies that address acute management issues for diabetes
* The post holder will lead the development of evidence based clinical nursing practice in line with current research and guidelines and will participate in audit.
* The post holder will work across organisational, regional and national boundaries and form good working relationships with colleagues within the regional diabetes network.

Qualifications

Education and Qualifications Essential

* RSCN/RN (child) and current NMC Registration
* Children & Young people's diabetes care module or equivalent qualification
* Teaching and Assessing qualification/mentorship course
* Current practice assessor and supervisor on the live database
* Evidence of ongoing continuing professional development
* Non-Medical Prescribing Qualification or willingness to undertake this

Desirable

* Management qualification
* Masters degree or equivalent
* Counselling qualification

* Evidence of ongoing professional self-development
