Job Overview
We are seeking a dedicated and compassionate Care Team Leader to oversee and support our night caregiving staff in providing exceptional care to our residents. The ideal candidate will possess a strong background in patient care, with experience in group homes, assisted living, or similar environments. As a Care Team Leader, you will play a crucial role in ensuring the well-being of our residents while fostering a positive and collaborative work environment.
Responsibilities
* Supervise and support caregiving staff in delivering high-quality care to residents.
* Develop and implement individualized care plans tailored to each resident's needs.
* Assist residents with activities of daily living (ADLs) such as bathing, dressing, grooming, and meal preparation.
* Monitor patients' vital signs and overall health status, reporting any changes to appropriate medical personnel.
* Ensure compliance with HIPAA regulations and maintain confidentiality of resident information.
* Provide training and guidance to staff on best practices for patient observation and care.
* Facilitate communication between residents, families, and healthcare providers to ensure continuity of care.
* Maintain cleanliness and organization within the living environment.
* Assist with medication administration as per care plans and regulatory guidelines.
* Engage residents in meaningful activities that promote social interaction and cognitive stimulation.
Skills
* First aid and CPR certification is essential.
* Experience in group homes, assisted living facilities, nursing homes, or hospice care is highly desirable.
* Strong skills in patient observation, monitoring vital signs, and providing direct support to individuals with developmental disabilities or Alzheimer's disease.
* Familiarity with memory care practices and dementia care strategies.
* Ability to assist with heavy lifting using equipment such as Hoyer lifts when necessary.
* Proficient in meal preparation and cooking for diverse dietary needs.
* Knowledge of home health services and long-term care principles.
* Excellent interpersonal skills for working effectively with residents, families, and team members.
