JOB [ Click Apply]db742659 Locum Social Worker - Physical Disability & Older Persons Location: Reading, UK Rate: £34.95 per hour Employment Type: Full-time (Locum) Contract Type: Ongoing Overview Join the Physical Disability and Older People's Team at Reading Borough Council and play a key role in delivering person-centred care through the Council's innovative Living Well Service. This is a fantastic opportunity to apply your experience in adult social care within a collaborative multidisciplinary environment, working alongside teams in Mental Health, Learning Disability, and Autism. Key Responsibilities * Carry out Care Act assessments to identify individual care needs and eligibility. * Develop tailored support plans, empowering individuals to maintain independence and well-being. * Conduct Mental Capacity Assessments and participate in related court work, including Court of Protection applications. * Lead adult safeguarding enquiries, ensuring safety and compliance with statutory duties. * Work collaboratively with health professionals, community teams, and external partners to deliver holistic care. * Maintain clear, concise records and contribute to ongoing team evaluations and service development. * Adopt a strengths-based approach to practice, championing the dignity and independence of each service user. Ideal Candidate Profile * Qualified Social Worker with Social Work England registration. * Strong knowledge of the Care Act 2014, Mental Capacity Act 2005, and safeguarding frameworks. * Demonstrated ability to conduct mental capacity assessments and manage complex safeguarding cases. * Experience in court work (desirable). * Strong interpersonal and communication skills for working with families, professionals, and multidisciplinary teams. * Ability to work independently and as part of a team in a fast-paced environment.
