Job Description
Age UK South Cumbria has to vacancies available for the position of: Home from Hospital Support Officer.
* Supporting local hospital teams in providing timely and effective discharges for older people
* Transporting older people home from hospital
* Ensuring a safe and sustainable transition home
* Conduct a holistic assessment of their needs
* Provide short term assistance to promote their independence
The two positions are based out from our Kendal office or Barrow-in-Furness office.
NB: No personal care will be provided by the post holder.
Full UK driving license essential.
