We have an exciting opportunity for a Hospital Liaison Team Leader who has the skills and experience to support and develop our Hospital Liaison offer in Birmingham. The successful candidate will manage a Hospital Liaison team based in the hospitals across Birmingham, supporting new and existing service users to engage with and maintain support from our community drug and alcohol treatment service.

No two days are the same and we treat every person who comes into our service as an individual, so being flexible, curious and a good listener are key.

Full time Hours: 37.5 hours per week

Full Time Salary Range: £35,220.59 - £37,184.90*

Full-time hours at Change Grow Live are 37.5 hours per week. For part-time roles, the salary and payments will be pro rata based on contracted hours.

Responsibilities

As a Hospital Liaison Team Leader, you will work as part of the service, developing and maintaining pathways between hospitals and our community treatment service in order to support seamless continuity of care. As the team leader the post holder will be expected to work with their team to support existing service users to maintain their treatment journeys whilst in hospital; as well as working with the alcohol care team, emergency department, etc within hospitals to effectively engage, triage and facilitate access to drug or alcohol treatment patients not currently accessing treatment to treatment service users. The team leader will also line manage a number of recovery coordinators working within Birmingham Hospitals and will link in with the community team managers to ensure the coordination of service provision.

Partnership Working: To work in partnership with all relevant hospital wards and teams develop pathways and mutual ways of working to gain the best outcomes for the service users. This will ensure patients are supported safely from hospital back into the community, with continuity of care throughout the transition

Service Development: Work with partners to increase accessibility into treatment for those presenting in hospital who have had problematic alcohol or drug use.

Leadership: To lead the hospital liaison team across Birmingham.

Developing Relationships: To lead on relationships with the Alcohol Care Teams and key hospital wards to ensure an effective pathway for service users on discharge from hospital into community services which includes a smooth transition.

Supporting Staff: To support staff to both effectively manage enquires and referrals from hospitals; ensuring liaising with community teams on discharges; and maximising opportunities to engage and triage patients whilst in hospital.

Data: To work with staff including data analyst as well as partners and to ensure data is captured and recorded appropriately.

Person Specification:

Understanding: Excellent understanding of drug and alcohol issues and experience of working within a hospital or substance treatment service.

Knowledge: Knowledge of the Criminal Justice system.

Collaboration: Experience of proactively developing and maintaining multidisciplinary partnership relationships
