We are looking to employ a dynamic individual to come and join our friendly team at Filey Surgery. We are a teaching and a training practice and encourage development of all our staff.
Main duties of the job
The practice nurse is tasked with delivering nursing services and works as a member of the practice's multidisciplinary team, providing care within their scope to the designated patient population. Responsibilities include various clinical areas including health promotion, effective management of patients with long term conditions, preventative health, wound management and any other primary care nursing including in our contract.

Job responsibilities
Primary responsibilities
The following are the core responsibilities of the practicenurse. There may be on occasion a requirement to carry out other tasks; thiswill be dependent on factors such as workload and staffing levels:
a.Develop, implement and embed healthpromotion and well-being programmes
b.Implement and evaluate individual treatmentplans for chronic disease patients
c.Identify, manage and support patients atrisk of developing long-term conditions, preventing adverse effects on patientshealth
d.Provide routine nursing care to patients asrequired in accordance with clinical-based evidence, NICE and the NSF
e.Provide wound care (ulcer/Doppler etc.) topatients
g.Request pathology services as necessary
h.Process pathology results as required
i.Provide chronic disease clinics, deliveringpatient care as necessary, referring patients to secondary/specialist care asrequired
j.Maintain accurate clinical records inconjunction with extant legislation
k.Ensure read codes are used effectively
l.Maintain chronic disease registers
m.Develop, implement and embed well womanclinics
n.Develop, implement and embed well manclinics
o.Chaperone patients where necessary
p.Assist GPs with minor surgery when required
q.Prioritise health issues and interveneappropriately
r.Support the team in dealing with clinicalemergencies
s.Recognise, assess and refer patientspresenting with mental health needs
t.Implement vaccination programmes for adultsand children
u.Support patients in the use of theirprescribed medicines or over-the-counter medicines (within own scope ofpractice)
v.Liaise with external services/agencies toensure the patient is supported appropriately (vulnerable patients etc.)
w.Delegate clinical responsibilitiesappropriately (ensuring safe practice and that the task is within the scopeof practice of the individual)
x.Support the clinical team with allsafeguarding matters, in accordance with local and national policies
y.Understand practice and local policies forsubstance abuse and addictive behaviour, referring patients appropriately
z.Deliver opportunistic health promotion whereappropriate
Secondary responsibilities
In addition to the primary responsibilities, the practice nursemay be requested to:
a.Engage in audit of services as and when required
b.Monitor and ensure the safe storage,rotation and disposal of medicaments
c.Support junior members of the nursing team,providing guidance when necessary
d.Participate in local initiatives to enhanceservice delivery and patient care
e.Support and participate in shared learningwithin the practice
f.Continually review clinical practices,responding to national policies and initiatives where appropriate
g. Participatein the review of significant and near-miss events applying a structuredapproach, i.e. root cause analysis (RCA).

Qualifications
* Registered Nurse
* Experience of chronic disease management
* Experience of working autonomously
* Respiratory Disease Management Diploma
* Mentor or teaching qualification
* Post graduate diploma or degree (Chronic Disease Management)
* Nurse Prescriber
* Experience of working as a practice or community nurse
* Experience of infection prevention and control measures
* Experience of managing elderly and housebound patients
