We are seeking a motivated and compassionate Basic Grade Occupational Therapist to join our Community Occupational Therapy Service in Cheshire East.
You will assess the needs of adults in the community who are ill or have a disability and, through the provision of specialist equipment, adaptations, and expert advice, help them to achieve greater independence and remain safely in their own homes.
Principal Responsibilities
* Assess the needs of adults with illness or disability for equipment or home adaptations, based on diagnosis and prognosis.
* Provide information, guidance, and professional advice to service users, carers, and partner agencies.
* Carry out assessments for individuals returning from hospital or residential care and provide equipment to support safe discharge.
* Where necessary, seek funding from other agencies (e.g., NHS, charities, housing) to support service user needs.
* Liaise with architects, service users, and contractors to ensure adaptations meet assessed needs and achieve the best outcomes.
* Collaborate with housing, health, and voluntary organisations to ensure coordinated service delivery.
* Monitor and evaluate completed adaptations and ensure equipment is correctly fitted and utilised.
* Maintain up-to-date knowledge of developments in Occupational Therapy equipment, moving and handling techniques, and best practice.
* Assess for potential rehabilitation and make appropriate referrals.
* Engage in Continuing Professional Development (CPD) and maintain an up-to-date professional portfolio in line with HCPC standards.
Requirements
* Degree or Diploma in Occupational Therapy.
* Current registration with the Health and Care Professions Council (HCPC).
* Experience in community-based assessments and provision of equipment/adaptations (desirable).
* Strong communication, assessment, and problem-solving skills.
* Ability to work independently and manage a varied caseload effectively.
