Job description The role of the Care Co-Ordinator includes all aspects included within the Support workers role. The role is to undertake those duties required of them as delegated by the Manager or Deputy Manager. The delivery of care will be determined by the individualised care plans and the Care Co-ordinator will report regularly to the Manager/Deputy Manager, passing on any changes that are observed or problems identified. As a Care Coordinator at LTS Social Care Services, Your responsibilities include participating in assessments and reviews, managing and supervising care workers, setting expectations for each shift, supporting service managers, arranging staff meetings, liaising with other professionals, maintaining records, reporting incidents, and carry out and compile a 2-week in advance rota for the staff team. As a care coordinator, you will aid in the training and development of other Care staff and update records accordingly. You will act as a role model of good practice for other care staff to follow and you will demonstrate commitment to the development of the company through interactions with staff, residents, and members of the general public. Lastly you will ensure that staff always follow policies and procedures and ensure staff have signed all related documents, whilst also ensuring that staff are working in line with the National Health and Social Care Standards. What we are looking for; Candidates must possess strong IT skills. available to work fulltime ideally hold an SVQ level 3 qualification or be willing to work towards the qualification. A valid drivers license and access to a car is necessary.
