Lincolnshire Community Health Service (LCHS) has an opportunity for Registered Nurses to join their Grantham Community Nursing Team. We have vacancies for 1 full‑time permanent position and 2 part‑time positions.
As a community staff nurse you will work autonomously and as part of a team, delivering care to patients in their own homes, care homes and clinic settings. Successful candidates will be responsible for assessing care needs, planning, delivering, and evaluating nursing care using evidence‑based practice.
Main duties of the job
Community nursing is exceptionally rewarding as it allows therapeutic relationships to be developed with patients in their own homes. The role embodies a person‑centred care approach and places patients at the centre of care delivery.
The skillset of a community nurse is vast and non‑exhaustive, requiring knowledge and expertise in a range of areas. Interventions include wound management, continence care, palliative care, medication administration, and long‑term condition management. Close collaboration with the MDT (social workers, GP's, Allied Health Professionals) ensures effective coordination of patient care, demonstrating a positive impact on our patients.

Job responsibilities
Travel to patients’ homes and clinic settings is required; access to transport is essential due to rurality.
The service operates from 8:00 am to 6:30 pm, seven days a week, with variable shift patterns: early shifts (8:00 am‑4:00 pm, 9:00 am‑5:00 pm) and late shifts (10:30 am‑6:30 pm). Weekend hours are 8:00 am‑5:00 pm or 8:00 am‑6:30 pm.
All new staff undergo a 12‑week induction programme, robust preceptorship, and clinical skills training, supported by an experienced clinical education team.

Qualifications
 * Current first‑level UK NMC adult nurse registration
 * Evidence of continuous professional development
 * Demonstrated mentorship, supervisory or assessor qualification and its application in practice
 * Further relevant training (e.g., palliative care, wound care, elderly care, diabetes)
 * Evidence of completed leadership modules/qualification
 * Evidence of V100 or V150 Community Practitioner prescribing course undertaken
 * Post‑registration qualifications (palliative care, care of the elderly, wound care, continence or other relevant post‑qualification)
Experience
 * Experience of assessment, care planning, and care delivery
 * Experience of delegating and supervising others
 * Application of research and evidence‑based care
 * Teaching or dissemination of knowledge and skills
 * Team working experience
 * Health promotion / coaching strategies experience
Skills
 * Range of clinical skills pertinent to area of practice
 * Excellent communication skills, including record keeping
 * IT and electronic patient record systems competence
 * Ability to perform practice audit
