Practice Nurse with a special interest in Diabetes
This is an exciting opportunity for a practice nurse with a special interest in diabetes to join a friendly, dynamic, and forward-thinking practice located East of Southampton.
Working as a member of the practice multidisciplinary team, the post holder will be responsible for the routine, evidence-based care of patients with diabetes, including annual reviews incorporating local prescribing guidelines as well as following up with patients with more complex diabetic needs,
The Practice Nurse will also be responsible for a number of clinical areas, such as infection prevention and control, health promotion, chronic disease management, and well-woman and well-man clinics, as well as actively supporting the practice management team in reviewing and delivering clinical policy and procedures.
Main duties of the job
Diabetic Reviews and Management
Wound care/removal of sutures and staples
ECGs
Venepuncture
Requesting pathology tests and processing the results, advising patients accordingly
Understanding the importance of evidence-based practice
Understand the requirement for PGDs and associated policy
Ability to record accurate clinical notes
Ability to work within own scope of practice and understanding when to refer to GPs
Chaperone procedure
Immunisations (routine, childhood and travel)

We also launched an online platformcalled eConsult, where patients can contact us digitally. All our rooms areequipped with video calling and double screen monitors.
We are active in the Bitterne Primary Care Network,with a partner having shared responsibility as a clinical director. Our PCNEducation Environment Lead also helps to ensure staff opportunities to meet anypersonal development goals.

Qualifications
* GCSE grade A to C in English and Maths
* EMIS Web / Docman 10 software experience
