What the Job Involves This role involves providing assessment, care management, and social work services to adults with mental health problems and their carers within a community setting. You will manage, monitor, and review care plans and collaborate with a multidisciplinary team. Responsibilities include serving as a care coordinator, participating in safeguarding adult enquiries, and utilizing information management systems. You must comply with local recording policies and be open to training, including as an Approved Mental Health Professional and Best Interest Assessor. You will undertake AMHP duties and be part of the boroughs AMHP rota.
