Job overview
An exciting opportunity for a Lead Nurse has arisen to join the busy and dynamic South and Dales OA CMHT.
We are looking for a clinician who has a specific interest and passion for the care of older adults.
The team provides assessment, treatment and care for patients who experience organic and/or functional illnesses, as well as support to their carers.
This role will primarily involve working into the Care Home project, independently carrying out light touch clinics with care homes, providing non-pharmacological advice and support, carrying a complex caseload of patients within the care home setting and carrying out assessment and intervention work alongside other clinical members of the team.
The post holder will also work into the duty rota, providing single point of access cover.
The post holder will have opportunity to develop leadership skills, and to have a role within the team of a link worker or champion (eg. student placement link worker, medicines champion, clozapine link worker)
Within the team, the Lead Nurse will work with other nurses, NMPs, Occupational Therapists, Psychologists, Nursing and Psychology Assistants, Consultant Psychiatrists, Junior Doctors, and Administrators to provide the best care for the population we support.
Main duties of the job
The role of Lead Nurse can be challenging and varied with the management of a complex caseload, completion of initial assessments, undertaking the role of duty worker, attending CST meetings and being part of the team's Care Homes Project.
The successful candidate will need excellent communication skills, the ability to complete thorough assessments, including safety assessments, be able to produce person-centred care plans with a living well/recovery focus and work within the CPA framework.

Detailed job description and main responsibilities
South and Dales OA CMHT is a friendly and dynamic team of experienced staff.
The successful candidate will receive clinical and management supervision on a regular basis, and will be supported to develop as a leader personally and professionally.
12. Carrying a complex caseload
13. Assessing service users with a functional/ organic illness and formulating care plans.
14. Undertaking duty; triaging referrals, communicating with other disciplines and signposting to the appropriate services

16. Being actively involved with the care homes project and CST meetings
