Salary: £22,932 per annum
Hours: 21 per week
Contract: Fixed term until end of September 2025
Driving Requirement: Full UK Driving Licence and access to a vehicle (Mandatory). Business mileage paid.

We are seeking a passionate and enthusiastic individual with excellent people skills to join our Health and Local Crisis Response service as a Service Support Worker. A day in the life of a Service Support Worker will involve:
* Providing practical and emotional support (telephone calls, welfare checks, shopping, etc.) and guidance to service users.
* Providing reactive, focused, and flexible support to adults following hospital stays to enable early discharge.
* Responding to referrals from clinicians and community health professionals.
* Assessing service users’ needs, completing, and following support plans for individuals.
* Supporting service users in the weeks after hospital discharge or to prevent readmission.
* Taking someone to hospital appointments or picking up prescriptions and shopping.
To be a successful Service Support Worker, you’ll:
* Have a full UK manual driving licence.
* Have good knowledge of NHS and Social Care services.
* Be able to improve service quality for users.
* Deal with queries professionally, diplomatically, and confidentially.
* Be flexible with working hours outside the norm.
