Duration, Location, and Working Pattern:
Full Time, Permanent
Base Location: Orchard View, Administration Office, Inverclyde
Shift Pattern: Monday to Friday, 9 am - 5 pm
Key Responsibilities
* Provide leadership and line management to an integrated discharge coordination team across health and social care
* Coordinate timely, safe and effective discharge planning for patients within inpatient mental health services
* Work collaboratively with multidisciplinary teams, social work, housing and partner agencies to deliver seamless discharge pathways
* Monitor patient flow, length of stay and discharge performance to support service improvement and quality assurance
* Undertake Response Nurse duties, including oversight of staffing, bed management and escalation of risks and incidents
* Lead on service development, policy implementation and continuous improvement of discharge processes and systems
* Support clinical governance, staff supervision, training and the delivery of best practice standards across the service
Knowledge, Training, Qualifications and/or Experience Required
* Registered Mental Health Nurse with current NMC registration
* Educated to degree level or equivalent with significant post‑registration experience at Band 6 or above
* Demonstrable leadership skills with experience of managing teams and complex clinical situations
* Comprehensive knowledge of mental health legislation, clinical governance and inpatient care pathways
* Proven ability to work autonomously, make sound clinical decisions and manage competing priorities
* Excellent communication, organisational and interpersonal skills with the ability to influence across services
* Evidence of continuous professional development and a strong commitment to improving patient care and service quality
