CLCH Hertfordshire Adult Community Services – CLCH NHS Trust is recruiting an Occupational Therapist with experience in stroke and neurological rehabilitation to join the Integrated Community Stroke Service (ICSS).
This is a 9‑month fixed‑term contract or secondment offering an exciting opportunity to work within a multidisciplinary team delivering high‑intensity and long‑term stroke rehabilitation.
Service Intensity
* Early Supported Discharge (ESD): high‑intensity rehab for up to 6 weeks post‑discharge for mild‑moderate new stroke.
* Neurological Enhanced Transition Team (NETT): high‑intensity rehab for up to 6 weeks post‑discharge for moderate‑severe stroke or other neurological conditions.
* Community Stroke Team: long‑term stroke rehab support.
Key Responsibilities
* Provide comprehensive occupational therapy service to residents within West Hertfordshire as part of the ICSS, supporting ESD, NETT, and the stroke community pathway.
* Assess, treat and manage an independent caseload of clients within the stroke pathway.
* Work within professional standards and clinical guidelines, promote best practice, and undertake all aspects of clinical duties as an autonomous practitioner.
* Build and maintain key relationships with referring services to facilitate appropriate referrals and support the coordinator where required.
* Lead occupational therapy activities, prioritising and delivering timely assessments.
* Supervise and line‑manage junior staff, including Band 6 occupational therapists, rehabilitation assistants, and occupational therapy students.
* Support service development through active participation in continuing professional development (CPD) and participation in any audit or research undertaken by the service.
Candidate Profile
The ideal candidate will have excellent communication skills, an adaptable approach, and be able to work confidently within stroke guidelines while contributing to the development of the stroke pathway. Experience in stroke and neurological rehabilitation is essential.
