Buckinghamshire Mind is looking for a Mental Health Safety Planning Outreach Support Worker.
The Mental Health Safety Planning Outreach Support Worker is responsible for supporting people’s wellbeing and safety following discharge from A&E or MHICS, and plays a key role in working with the East Berkshire Psychological Medicines Team and the Mental Health Integrated Services to accept referrals for individuals with suicidal ideation and/self-harming thoughts who have been assessed as not requiring secondary mental health care but highlighted as needing further support in the community.
The purpose of this role is to provide a short-term community and remote based intervention to provide listening support and develop Safety Plans with Service Users, and in addition, aid the Service Users to locate and utilise support networks both within and external to the community.
The Mental Health Safety Planning Outreach Support Worker will work in collaboration with other community resources and must have a skill set in developing relationships to achieve the desired outcomes of:
* Promote connection and reduce isolation
* Improve coping skills
* Signpost to other services and sources of support
* Provide educational strategies to help with difficulties that triggered self-harm.
* Contract Type: Fixed Term Maternity Cover ending 31st March 2026
* Location: home-based, with weekly travel to Wexham Park Hospital and occasional travel in the East Berkshire community
* Hours: 28 hours pw. Mon-Thu or flexible working options will be considered
* Annual Salary: FTE: £24,333.75. Pro Rata Actual: £18,414.73

Key Responsibilities
* Actively supporting Service Users to develop resilience in managing current or future distress. Creating a calm and compassionate person-centred congruent environment to build trusting relationships and empower them to build robust safety plans and encourage them to engage with relevant community services.
* Ensure the wellbeing and safety of service users, reporting any risk or safeguarding concerns to your line manager and organisational safeguarding leads in line with current policies and procedures.
* Liaise with internal and external teams to ensure collaborative working which ensures the service user is at the forefront, to ensure appropriate support and consistent approach to crisis management.
* Work within Buckinghamshire Mind’s and Berkshire Mind’s policies and procedures including Performance Management, Confidentiality, Safeguarding, Equal Opportunities, Service User Involvement, Health and Safety and the Professional Boundaries policy.
The ideal candidate will have
* A’ level qualifications or equivalent, or similar level of education gained through CPD.
* Experience of working with adults in the mental health sector or in a similar support or care giving role.
* Knowledge of social and situational factors influencing adults’ mental health, recovery and wellbeing.
* A positive attitude towards people with mental health problems, in particular people’s ability to develop and change.
