Care Navigator duties:
* Maintaining and monitoring the practice appointment system i.e. make/cancel/rearrange appointments via the clinical system
* Data entry of new and temporary registrations and relevant patient information as required
* File patient medical records, results and letters
* Process personal, telephone and e-requests for appointments
* Answer incoming phone calls, transferring calls or dealing with the callers request appropriately
* Signpost patients to the correct service
* Ensure up to date patient information is recorded
* Greeting patients at reception, noting their arrival time
* Observe the waiting room
* Ensure the waiting and reception areas are kept tidy
* Ensure effective communications, regularly updating the Team Leader of any issues
* Initiating contact with and responding to, requests from patients, team members and external agencies

* Ensure confidentiality at all times
* Maintaining the safety of staff, patients and visitors
* Maintain a clean, tidy, effective working area at all times
* Carry out any other duties as reasonably requested
Admin' duties:
* Action incoming correspondence when necessary
* Scanning of patient related documentation and attaching scanned documents to patients healthcare records
* Taking instructions from medical experts
* Performing general administrative duties such as scanning, photocopying and emailing
* File patient medical records, results and letters
* Ensure up to date patient information is recorded
* Input data into the patients healthcare records as necessary
* Direct requests for information i.e. SAR, insurance / solicitors letters and DVLA forms to the administrative team
* Manage all queries as necessary in an efficient manner
* Support all clinical staff with general tasks as requested
