Job Overview
An exciting opportunity has arisen for a Band 6 Community Psychiatric Nurse to work within the Conwy Community Mental Health Team for Older Persons.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
Main duties of the job
The post involves working with older clients presenting with symptoms suggestive of organic illnesses (such as dementia and cognitive impairments) and functional illnesses like anxiety and depression. The CPN will be responsible for assessing, implementing, and evaluating care plans and managing risks for persons over 70 living in the Conwy area. The role requires close collaboration with the Consultant Psychiatrist, Social Services, inpatient wards, and third sector and voluntary agencies to ensure appropriate care is provided in the right environment.

Person Specification
Qualifications
* Essential: NMC Registered Nurse Part 3 or Part 13
Desirable: Diploma or Degree in relevant subject
Experience
* Essential: Significant post-registration experience at Band 5 or above
Desirable: Experience within relevant clinical areas
Knowledge
* Essential: Up-to-date knowledge of the Mental Health Act and Mental Health Wales Measure
Desirable: Evidence of further relevant professional development
Skills
* Essential: Ability to work autonomously and in a multidisciplinary team
Desirable: Research skills and evidence of people management and leadership skills
